1. Valid passport information page, latest visa page or residence permit page;
2. Employment contract or certificate of employment (including the dispatch letter from multinational company);
3. 1x1 inch electronic ID photo (white background, taken in the past 6 months);
4. Original highest degree (education) certificate or relevant approval documents and professional qualification certificates;
5. Work qualification certificate;
6. Certificate of no criminal record;
7. Physical examination certificate within 7 or 6 months (go to Anhui International Travel Health Care Station Center at No.367 Wuhu Road with two 2x2 inch white background ID photos & passport, empty stomach, arrive before 11:00 am);
8. Please provide relevant materials of accompanying family member if any;
Passport information page of accompanying family member: proof of family relationship (marriage certificate, birth certificate of children, etc.), which shall be certified by the Chinese consulate before coming to China; electronic photo.
